The KYM Team from Simpang Empat Health Clinic which comprised of a pharmacist and a few DKUA had spent some time visiting a few patients in Simpang Empat, Perlis to assess the patients' knowledge and practices towards quality use of their medicines at home. There are some pharmaceutical care issues had been identified and proper counselling and medication-taking aides were given to the patients during the visit.
